Planetary Drive head keeps falling out during use

At some point the planetary drive head started wanting to just drop off (fall) - like, into the mixing bowl) when operating the mixer. Why does that happen? Does something need to be replaced? Did a clip or retainer disappear?  I could epoxy that drive head to the vertical shaft (the one that sticks down with MANY tiny long teeth along the shaft, but that might affect repairability long term.
