Your phone is about three years old now, so it's quite possible your battery is on it's last legs. As batteries age, especially if they are used heavily, the chemical reaction that powers the charge/discharge cycle begins to fail resulting in less ability to hold a full charge. It's entirely normal but can vary widely among various batteries, even on the same model of phone.

If you're somewhat new to phone repair, I'd suggest the Fix Kit version; for an extra $7 USD you also get all the tools needed to do the job, along with the adhesives to glue down that battery again and seal the rear cover back on.
